Complete Page 1 and the top of Page 2 (in CAPITAL letters using BLACK ink).
Do NOT sign or date the form until instructed by the Civil Surgeon.

2 - Government-issued photo ID

(Passport or Driver’s License).
Children under 14 should bring identification showing name, date/place of birth, and parent’s full name (birth certificate with English translation or affidavit).

3 - Vaccination records (if available)

Including DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, Hepatitis A & B, Varicella, Pneumococcal, Influenza, Rotavirus, and Meningococcal vaccines.
Records in another language must be translated into English.

4 - Hospital or clinic records

 Including documentation of prior tuberculosis treatment or infectious diseases.

5 - Prior chest X-rays or diagnostic reports

6 - Mental health records (if applicable)

7 - Alien Registration Number (A-Number), if available
