Visit 1

Your second visit is with the Civil Surgeon to:
- Complete your physical examination

This appointment typically occurs 5–10 days after your first visit, allowing time for lab results. Additional visits may be required depending on test findings or vaccination needs.
Please arrive 30 minutes early to complete paperwork. During the exam, the Civil Surgeon will review your medical history and perform a full physical evaluation. Once all lab reports are finalized and reviewed, you will be notified when your sealed I-693 packet is ready.
What to Bring to Your Appointment
1 - Form I-693
Complete Page 1 and the top of Page 2 (in CAPITAL letters using BLACK ink).
Do NOT sign or date the form until instructed by the Civil Surgeon.
2 - Government-issued photo ID
(Passport or Driver’s License).
Children under 14 should bring identification showing name, date/place of birth, and parent’s full name (birth certificate with English translation or affidavit).
3 - Vaccination records (if available)
Including DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, Hepatitis A & B, Varicella, Pneumococcal, Influenza, Rotavirus, and Meningococcal vaccines.
Records in another language must be translated into English.
4 - Hospital or clinic records
Including documentation of prior tuberculosis treatment or infectious diseases.
5 - Prior chest X-rays or diagnostic reports
6 - Mental health records (if applicable)
7 - Alien Registration Number (A-Number), if available
